The Most Head-Turning Face Transformations In Bravo Real Housewives History

Real Housewives are notorious for their drastic face transformations, though a few have purposefully chosen not to alter their looks. This select group includes Pettifleur Berenger of "The Real Housewives of Melbourne," who told Daily Mail, "I'm 100% natural. I've not had anything touched on my face, contrary to what people are talking about." However, we've seen countless Bravo stars become completely unrecognizable compared to their pre-fame or early Real Housewives days, and some have been more open than others about why they look so different.

Many side-by-side pics show how plastic surgery has transformed Real Housewives stars. But that's not the only reason many of these leading women look different these days. From non-invasive beauty treatments to illnesses to weight loss, these radical Real Housewives' face transformations are truly head-turning.

NeNe Leakes' two nose jobs and heavy makeup changed her face

NeNe Leakes' face looked totally different in 2026 compared to 2008. The iconic former "The Real Housewives of Atlanta" star has confirmed getting two nose jobs (aka rhinoplasty), with her first in 2010. "I still wanted to look like NeNe, the Black woman that I am but a better version," she told People at the time. But she looked different after the second. Appearing on "Watch What Happens Live with Andy Cohen" in 2016, she shared (via Bravo), "I didn't get a full rhinoplasty the first time. No, when I did the first time, I only had my nostrils taken in on the side ... and then this time, I had more cartilage moved around in my nose."

She also opened up about how she's changed her face on "The Breakfast Club" in 2026. "I am a walking filter," she admitted. The former "Glee" actor denied having invasive procedures on her face, such as a facelift, but admitted, "I've had liposuction. I do things to nip and tuck. Fillers, Botox, why not? I mean, I have the money to do it."

In 2024, Leakes' evolving face sparked a discussion on Reddit's r/BravoRealHousewives. One user claimed, "Don't recognize her anymore." Another suggested that her face transformation might be a result of evolving beauty trends — makeup trends have changed drastically over the years, after all. "She/her makeup artist are following makeup trends that were directly influenced by drag – specifically the heavy eyebrows and heavy contouring."

Lisa Rinna's silicone-filled lips and Botox contributed to her new look

Lisa Rinna's face transformation between 1989 and 2026 is anything but subtle. Despite the snaps being taken more than two decades apart, the actor hadn't shown many obvious signs of natural aging. Her skin looked smoother and plumper in 2026, while her already full lips had become even larger. On "Today" (via AmazingNewsVid), Rinna shared she had silicone injected into her lips when she was 24, but confirmed she'd had much of it removed. "They get hard. I mean, it's gross," she explained. "So, they are now just whatever that was after they took out as much of the silicone as they could," she added, branding herself a lip filler "pioneer ... one of the first ones to ever do it."

However, the head-turning full beauty transformation of Lisa Rinna proves that's not all she's had done. In her book, "You Better Believe I'm Gonna Talk About It," she got candid about Botox injections (which can reduce fine lines and wrinkles) and SKINVIVE treatments (where a hyaluronic acid gel is injected into the skin to smooth and hydrate the face), but shared that the two didn't give her the desired results. "Well, the combination of the Botox making my jawline atrophy, and SKINVIVE giving me volume, made me look freakish," she wrote (via Daily Mail). The star is also a self-confessed beauty junkie. "I really love the beauty business so I'm always open to trying new things that sound good," the former "The Real Housewives of Beverly Hills" star told People, suggesting she has no plans to stop experimenting with facial treatments.

Brandi Glanville's face underwent a drastic transformation due to a parasite

Brandi Glanville has been transparent about why her face has totally transformed, making her look unrecognizable in 2026 compared to 2006. Glanville denied getting invasive cosmetic surgery on X in 2023, writing, "I've NEVER had surgery on my face. I'm so sick of all the mean comments. I'm 50!" However, she added, "I do plan on getting a lower face & neck lift like [Teddy Mellencamp] at some point soon." But before the former "The Real Housewives of Beverly Hills" star could get that surgery, her face changed drastically due to a parasite that developed after she failed to replace her breast implants despite difficulties. "That's what caused the infection in my face; it couldn't get out as my lymph nodes were all clogged," she told TMZ.

The infection dramatically altered her facial shape, as it often switched between swollen and gaunt. "I prefer [my face] when it's a little swollen, to be honest," she told Page Six in 2025. "When it's only swollen on one side, it's not great. But the sinking in thing is killing me slowly ... I've had days of looking normal, not weeks. I wish. But it comes and goes," she added. A year later, she confirmed on X that she's had the facial fillers dissolved.

The reality star also removed her ruptured breast implants in 2026 and underwent facial cosmetic procedures following the health ordeal. "I'm not getting plastic surgery. Dr. Dorfman is doing like lasers and different things because it sucks to age 20 years overnight," she told Daily Mail. The star also shared on Instagram that she'd had CellSound treatments, which can help with repairing collagen cell elasticity.

Erika Jayne's face has transformed due to her serious dedication to glam

Before Erika Jayne found fame on "The Real Housewives of Beverly Hills," she wore less makeup, as seen via the minimal contouring around her cheekbones and jaw in the 2017 photo. The star, who also goes by Erika Girardi, had a more prominent-looking chin than in a 2026 Instagram photo, which may be the result of weight loss. Jayne hasn't publicly confirmed she's used GLP-1, but addressed her weight loss on "Today" in 2025. "I've taken care of myself as best as possible with the resources that I have," she said. However, she is among the TV stars who have sparked Ozempic rumors.

She's also been honest about her nose job, made evident by its slimmer appearance in 2026. During an interview on "Watch What Happens Live" in 2018, host Andy Cohen shared a photo of Jayne with her original nose. "That was some bad hair and makeup!" Jayne admitted (via Bravo). After Cohen stated she was almost unrecognizable in the snap, Jayne replied, "It was the same face, just a different nose."

Then there's the insane amount the reality star spends on beauty treatments. On "The Wendy Williams Show" in 2018, she admitted she could spend up to $40,000 a month on her looks. "If you want to look good it's hair, it's makeup, it's wardrobe, and then those people that help facilitate that. They're not cheap," she said (via People). She also dished on her beauty routine with Yahoo! Entertainment in 2025, admitting, "Yes, there are lasers, and yes, there's Botox, and yes, there's all of this stuff to basically just stay camera-ready."

Phaedra Parks' face shape has changed over the years

Phaedra Parks' face has become noticeably less round over the years, and she had a much more prominent chin in 2025 than when she posed for the cameras in 2007. That could be down to Parks' weight loss, as she told Hollywood Life in 2021 she'd lost 18 pounds due to intermittent fasting and exercise. She's also been open about her healthy diet. "I'm really big on healthy skin under makeup. Healthy skin starts within so I really try to maintain a great diet. I drink no less than a gallon and a half of water every day. I like to mix fresh lemons and ginger in my water, sometimes a little cucumber," she told Essence in 2020.

Her skin appeared just as smooth as years earlier, and her face was devoid of the obvious natural lines and wrinkles that come with aging. The ex-"The Real Housewives of Atlanta" star has denied that plastic surgery is the reason, telling Beverly Hills Rejuvenation Center in 2024, "While I'm not opposed to people going under the knife, I am going to fight that tooth and nail until I can't. I love machines because there are so many cutting-edge new ones." The star has hinted at using Botox to achieve her healthy-looking skin, though. She told The U.S. Sun she gifted guests who attended her 50th birthday facials and Botox, sharing, "These little tricks are important because we plan to be around looking and feeling fabulous for at least another 50 years!"

Kim Zolciak said her new look is down to Botox and skin tightening

In 2009, Kim Zolciak had a more prominent nose that didn't appear quite as smooth as seen in a 2026 Instagram selfie. Her nose also seemed to have a thinner bridge and a more upturned end, which inspired plenty of nose job chatter. Andy Cohen teased Zolciak about the rumors on "Watch What Happens Live with Andy Cohen" in 2016, when he made her pose alongside a snap from "The Real Housewives of Atlanta" season 1. "So you don't think that nose is a little different, Kim?" he asked. She responded, "No." Cohen then quipped, "The tip is not different? It's kind of hanging down differently?" Zolciak put her altered-looking nose down to makeup and admitted, "If I'm going to have a nose job, it's going to be a hell of a lot shorter, okay? For real. It's not going to go down into my mouth when I smile."

Though some Reddit users have disputed the ex-"The Real Housewives of Atlanta" star's nose job denial, Zolciak has shared a few other cosmetic treatments that could be behind her brighter skin and rejuvenated look. On "Botched Presents: Plastic Surgery Rewind" (via E! News) in 2025, the star, one of the celebs you wouldn't recognize without lip filler, revealed, "I did have filler in my lip. I do Botox all the time." She also copped to getting Ultherapy (which aims to lift and tighten the skin) on her jaw in a 2016 Bravo interview, and revealed in a 2023 YouTube video with Dr. David Matlock that she'd lost 20 pounds. That may explain why her 2026 selfie showed a longer-looking face.

Margaret Josephs has been open about the plastic surgeries that have transformed her face

In 2019, Margaret of "The Real Housewives of New Jersey" shared a throwback Instagram photo in which she looked almost unrecognizable. She didn't confirm when the snap was taken, but she appeared to be in her 20s. Compared to a 2026 Instagram upload, her nose appeared thinner and more defined with a slightly longer tip, while her eyebrows had a noticeably more prominent arch. Despite potentially 30 years of aging, which had caused a few lines around her eyes and jaw, Josephs' skin still had a youthful glow.

Josephs revealed in 2023 that she'd lost 22 pounds via peptide and hormone replacement therapy, which may have contributed to her less-rounded jaw. She told People that she found her weight loss transformative. The TV personality also confirmed she'd had a nose job in a 2026 Instagram post, writing, "With age the cartilage in my nose didn't support my tip and it emphasized a slight bump in my nose also. Every photo of me I really noticed it and it bothered me." In the same post, she told fans she'd also had fat transfer surgery and stem cell therapy. "It was to restore volume in the face too," she wrote. Those treatments followed a facelift she absolutely loved. Josephs also lasers her skin and gets Botox regularly. Check out the health benefits of Botox.

Larsa Pippen's face looks totally different than when she first found fame

Larsa Pippen joins our list of Bravo stars who are completely unrecognizable now. In 2009, her nose was longer, her cheeks weren't so full, and she had thinner lips. In 2026, she also wore fuller, longer, faux lashes, which drew more attention to her eyes, while heavier contouring emphasized her cheekbones. But more than just a makeup change has contributed to her evolving look.

During 2022's "The Real Housewives of Miami" season 4 reunion, Pippen confirmed (via NewBeauty), "I've had my nose done. I've had my lips done and that's basically it." She also put her new look down to weight loss, sharing, "If I show you my photos from five years ago, I was less than 100 pounds. I'm 140 pounds now ... my whole body's changed." Three years later, she explained her puffier face on Instagram Stories. "I did have PRP (Platelet-Rich Plasma). I did have an allergic reaction to the PRP, which I didn't even know you can have, so my face is a bit swollen," she said (via Page Six). The treatment involves having blood drawn and run through a machine that separates the platelets, then reinjected into the skin. It can reduce wrinkles and encourage plump skin.

Her radical transformation has spurred a lot of chatter online. After seeing a photo of the star from 2011, one Redditor wrote in r/BravoRealHousewives in 2022, "If I didn't know it was Larsa, I would honestly think these were two separate women. I know people say that a lot but in this case it's true ..." Another commented in the subreddit, "Wow. It's like a completely different person ..."

Dolores Catania's face has less obvious lines and wrinkles than 10 years earlier

Dolores Catania of "The Real Housewives of New Jersey" and "The Real Housewives of Rhode Island" had more noticeable lines on her face in a 2016 photo compared to one from 2026. Her skin also appeared less plump and smooth, while the latter snap showed her eyes with a more elongated, cat-eye shape. Catania confirmed on Instagram in 2019 that she'd had a facelift, necklift, and eyelid surgery, writing, "No one ever believes that I had a facelift when I tell them because I still look like me." She also revealed in a 2024 Instagram post that she'd lifted her temples and upper cheeks.

Speaking to Bravo's The Daily Dish about why she's been so open about her multiple surgeries, she said, "Well, I feel a responsibility to people. We're like a community. I feel like it's only fair for them to know that no, this isn't just me. I was not born like this. I had help and I looked into what was and the best way to do it." She added, "So when I do the homework and I share it, for me, it's like part of my job to do that. To be honest and be transparent."

Catania has also credited less invasive procedures for her ever-evolving look. "I'm a bit of a beauty-product addict—I just can't get enough of it," she told New Beauty in 2024. Among her go-to products? LED masks, face scrubs, and moisturizing night creams.

Porsha Williams looks different because of a non-surgical nose job and focus on her health

Former "The Real Housewives of Atlanta" star Porsha Williams has changed a lot, as seen in an Instagram selfie from 2026 compared to how she looked in 2013 (the year after she joined the Bravo show). In the more recent snap, the bridge of her nose appeared straighter, and her face seemed longer and less full in the cheeks. Williams' lips were plumper, and she had thicker brows, too, which helped change the shape of her face. The star also opted for lashes that were more fanned out and longer at the ends, which helped open up her eyes.

Her face transformation is partly down to a non-surgical nose job, which altered the shape of her nose using filler. Williams documented the process via an Instagram video in 2016. In 2020, she announced she'd gotten Botox as a Mother's Day gift to herself. "She gave me Botox on my forehead and some cheek highlighting!" Williams wrote of her injector on Instagram

The notorious reality star's face may also have changed due to weight loss. In 2024, she shared an Instagram video of herself on the treadmill and confirmed in the caption that she'd been taking her health more seriously. She also noted she'd started meal prepping and drinking more juices while cutting out alcohol, which followed her telling Bravo's Chat Room in 2020 that she'd adopted a predominantly vegan diet.
