Side By-Side Pics Show How Plastic Surgery Transformed These Bravo Real Housewives

A lot of Bravo stars look unrecognizable compared to when they made their debut on the reality TV network, but few are quite as obvious as the "Real Housewives." Plenty of members of the hugely successful reality franchise have gone under the knife, so much so that it seems like breast augmentations and nose jobs are a right of passage to be cast on any of the series. But while nips and tucks may once have been considered closely guarded secrets, several wives have been open and honest about their procedures. In fact, some have even shared before and after shots with their followers to show off their changed body parts.

For instance, in 2022, former "The Real Housewives of Beverly Hills" star Teddi Mellencamp got candid about her rhinoplasty (aka nose job) on Instagram. Mellencamp (who's also used her platform to inspire us with her powerful cancer journey) posted a series of photos showing her before and after side profile. "I always want to be as transparent as possible here on the gram," she wrote. "I chose at 40 to do this because I wanted to," she added. And she's far from the only Real Housewife whose side-by-side pics show a dramatic plastic surgery transformation.

Though "Real Housewives of New Jersey" star Melissa Gorga initially denied getting rhinoplasty, she came clean about her work in 2016 after her former co-star Jacqueline Laurita spilled the beans. Gorga acknowledged one nose surgery but denied that she'd had four procedures as Laurita claimed, telling Bravo's The Daily Dish, "There was definitely not four nose jobs." Still, the single surgery dramatically changed Gorga's face, as the bridge of her nose appeared far slimmer in 2025 compared to snaps from 2011 (around the time she made her "RHONJ" debut). Prior to that, Gorga's former friend and "RHONJ" guest star Jan DeDolce told All About The Real Housewives that Gorga's makeup artist had been heavily contouring the New Jersey native's nose. 

Gorga's skin also arguably looked more taut in the 2025 pic. That may be down to Botox, as Gorga confirmed on "Jeff Lewis Live" in 2023 (via The Daily Dish): "Obviously, since Season 3 [of "RHONJ"], I've had a nose job — everybody knows this. I've definitely done Botox, I've done fillers."

Gorga is also a celeb who's been open about her breast augmentation. She confirmed on Instagram in 2015 that she first had the surgery 13 years earlier, but was changing her implants (it's recommended to replace breast implants every 10 to 20 years). She then told her Instagram followers in 2021 that she'd undergone a breast reduction by the same surgeon. "She has triple D's and they're going down to C's!!" Gorga wrote, referring to herself underneath a snap showing her in a hospital gown alongside plastic surgeon Michael Fiorillo.

Larsa Pippen made her debut on "The Real Housewives of Miami" Season 1, but when she returned to the show for Season 4, she looked strikingly different. During the reunion, Pippen confirmed she had a rhinoplasty and noted (via The Daily Dish), "I've had my lips done. And that's basically it," though she also revealed had a breast augmentation. But judging from the photos, the beauty evolution didn't stop there. A 2025 photo of Kim Kardashian's former bestie next to a 2012 snap shows the bridge of her nose is thinner, while the tip looks more upturned. Her lips have also undergone a drastic transformation, as her upper lip in particular is much plumper than when she initially courted reality TV fame.

Pippen's new look may also be down to platelet-rich plasma therapy (also known as PRP injections). She shared she had a negative experience with the procedure after fans noticed her face appeared swollen in August 2025. "I did have an allergic reaction to the PRP, which I didn't even know you can have, so my face is a bit swollen," she wrote, in part, on Instagram Stories (via Page Six).

Kyle Richards is the only "The Real Housewives of Beverly Hills" star to appear on every season, so long-time viewers have had a front row seat to her complete plastic surgery transformation. One of the procedures that changed Richards' look most drastically was her rhinoplasty. "My nose bothered me for a long time. The surgery made me feel better," she told Us Weekly in 2013. Since then, the star has denied getting any other procedures on her face. "I have not done plastic surgery. I did my nose in 2006 and I get Botox. That is it," she wrote on Instagram Stories in 2019 (via Us Weekly). However, the following year she shared that she'd undergone a second nose job for medical reasons. "The bone poking out bothered me. So I fixed the bone, fixed my breathing problems and refined the top," she wrote on Instagram Stories (via People). Her plastic surgery evolution is clear in a photo from 2025 next to one from 2004, where Richards' nose looks more prominent.

The actor hasn't been a gatekeeper when it comes to her changing body either. In 2013, she confirmed on Bravo's "Secret's Revealed" she'd had liposuction. "After having four kids, no matter what exercise I did, the love handles wouldn't go away," she said. " ... I'm very happy with the results." The actor and reality star has also joined the ranks of celebrities who have proudly spoken about their breast reduction surgeries. "I've never had implants, [but] I had double Ds. ... I went in and I said, 'I want smaller boobs. I don't ever want to wear a bra again in my life,'" she said on "Watch What Happens Live With Andy Cohen."

Former "The Real Housewives of New Jersey" star Jacqueline Laurita has been extremely open with fans about her various plastic surgeries, documenting her transformation on the show and on social media. She told People in 2017 she already had four breast augmentations, while "RHONJ" cameras captured her tummy tuck and neck lift on Season 5. Several years later, in 2025, she kept fans fully updated with what appeared to be her second neck and face lift on Instagram. "My 5 hour surgery went great with [Dr. T. Y. Steven Ip, MD] ... SO FAR I can't feel a thing! I feel great! I'm still waiting for the other half of my face to wake up that was worked on last. Should be soon!" she wrote, in part, alongside a video showing her with bandages around her head and what appeared to be an oxygen tube in her nose. A few years before that, Laurita documented her visit to a Las Vegas clinic where she got less permanent cosmetic procedures (including cheek filler, jawline filler, and a liquid nose job) on Instagram Stories. Since then, Laurita's skin, including on her neck, has appeared smoother and tighter, while her chin area looks more defined.

Wendy Osefo's plastic surgery was much-discussed on Season 6 of "The Real Housewives of Potomac" — and let's just say, the star hasn't made any attempts to hide it. Osefo confirmed she got a Brazilian Butt Lift and a breast augmentation, showing off the results during the Season 6 premiere of the Bravo series. "I breastfed all three of my children for over a year, and that takes a toll on your body. So, this is one of the first things that I've ever done for myself," she explained (via Daily Mail). Osefo later admitted she felt body shamed by her co-stars for changing her look. "People have plastic surgery and this is part of why people don't say stuff," she said (via The Daily Dish). 

Despite the criticism, the star seems happy with her decision. "Imagine me worrying about what another woman wears or does with her body?! ... I can't relate," she wrote alongside a snap of herself in a one-piece swimsuit shared to Instagram. Her plastic surgery transformation is clear when comparing a photo of her taken in 2019 to one taken in 2025. Osefo appears far more confident showing her chest following her enhancement, which made her noticeably more hourglass-shaped.

"The Real Housewives of Orange County" star Tamra Judge's face changed a lot between 2010 and 2025. Judge still had smooth skin (despite there being 15 years between the two snaps) in the more recent photo, while her brows had a higher arch that gave her face a more elongated appearance. That may have been down to the multiple plastic surgeries, which she hasn't shied away from confirming. "I visited Dr. Donald Mowlds in Newport Beach ... for a brow lift, CO2 laser, and a blue chemical peel," Judge wrote on Instagram in 2024, alongside several photos and videos of her pre- and post-surgery. "Seven years ago, I had a lower facelift but skipped the upper — like painting the house but forgetting the trim," she wrote.

Speaking to Real Self about why she chose to be so candid about her surgeries and her recovery, she shared, "I want to be transparent and show other women that they don't have to be afraid or ashamed. And if they're thinking about doing this, then this is the process and what it will look like day to day." The Real Housewife added, "Nobody documents right after surgery, they show the before and the after, so I was happy to show exactly what the whole journey is like."

Judge also addressed her changing looks on "Watch What Happens Live with Andy Cohen" in 2025, denying she'd had further plastic surgery. "Everyone's, like, 'What? You look different.' I think even Andy [Cohen] said that. I'm, like, 'My hair's darker. I'm definitely thinner,' which makes a difference, I think, in your face," she said (via The Daily Dish), sharing she also got new veneers. "That's it, guys. No surgery, no nothing."

A snap of Vicki Gunvalson taken in 2009 compared to one of her in 2025 shows drastic changes to her nose and face shape, as well as a more prominent upper lip and raised eyebrows. Gunvalson has confirmed she's had a lower face lift, as well as plastic surgery on her nose, chin, breasts, and upper and lower eyes. In 2013, she spoke openly about a few of her procedures, stating on "The Real Housewives of Orange County" (via People), "I did some cartilage reconstruction on my nose. I had some fat injected in the upper part of my cheeks so I don't have any more wrinkles. And I did a little chin implant." But Gunvalson admitted to People in 2017, "I do regret having my nose and my chin done two weeks before filming four years ago." The reality star added, "I was so swollen and I couldn't move my mouth and it was totally embarrassing."

In 2018, Gunvalson went under the knife again. That time, she got a lower face lift after feeling inspired by one of her "RHOC" co-stars. "I went to Tamra [Judge]'s doctor, 'cause she told me I needed a face-lift," Gunvalson told ET, but claimed she was done with plastic surgery following the procedures. However, in 2025, the reality star was also featured on Dr. T. Y. Steven Ip, MD's Instagram (the same surgeon who treated Jacqueline Laurita) for correction of capsular contracture surgery, which appeared to be done on her chest. 

Back in 2009, "The Real Housewives of Atlanta" legend NeNe Leakes looked totally different than she did in 2025. For one, the shape of her nose had noticeably changed, while her skin appeared smoother and more taut, making for one of the most striking transformations of any Bravo star. Leakes is a celebrity who's owned her cosmetic tweaks, including her decision to undergo two rhinoplasty surgeries. In her case, though, it wasn't always about looks. "It was a real medical reason why I had to get it done again. My cartilage was growing in my nose and my tip was touching the top of my lip. The tip of my nose was touching my lip when I smiled," she said on "Watch What Happens Live with Andy Cohen" (via Page Six).

But it's not just her face that's had a nip and tuck. Leakes also went under the knife for a Brazilian Butt Lift in 2022. "When you say BBL, I think you're getting ready to come out of there with a big giant booty and I just want to fix my problem areas. So we called it a...mini BBL," she wrote in a since deleted Instagram Story (via RealSelf). That came after Leakes spoke to People in 2010 about replacing her breast implants with a smaller size, as well as undergoing a breast lift and liposuction. "I'm very comfortable and confident in myself. I just wanted a tune up," she explained. "I still wanted to look like NeNe, the Black woman that I am but a better version." 

It's impossible to deny that the 2026 Erika Jayne (aka Erika Girardi) looks almost unrecognizable compared to the 2007 of herself. For one, the star appears to have more prominent cheekbones, tauter skin, and a slimmer nose. Though she hasn't confirmed getting multiple drastic surgeries to change her look, Jayne underwent rhinoplasty aged 41. The star admitted to the procedure on "Watch What Happens Live With Andy Cohen" after host Andy Cohen shared a throwback photo of her taken in 2010. "I do remember [this]. That was some bad hair and makeup!" she said (via Bravo), before noting, "It was the same face, just a different nose." Erika Jayne has also shared how she's altered her figure. In her 2018 book "Pretty Mess," the singer confirmed she got a breast augmentation.

Erika Jayne has also gotten candid about the non-surgical ways she's changed her appearance. When asked by Yahoo! what she gets done to stay looking so young, she jokingly responded, "All of it. And the older I get, the more extensive it becomes." She added, "Yes, there are lasers, and yes, there's Botox, and yes, there's all of this stuff to basically just stay camera-ready." In an Instagram post, Beverly Hills medical spa Motykie Med Spa shared a few other treatments the star had received. "From Morpheus8 to skin-boosting treatments, neurotoxin, and fillers, it's always a joy helping her look and feel her best," the company wrote.

"The Real Housewives of New Jersey" stalwart Teresa Giudice's 2008 breast augmentation was filmed for Season 1 of the Bravo reality show, so you can bet she isn't shy when it comes to talking about cosmetic work. "I was confident before ... I liked [my breasts] being little, but after I breastfed they completely went away. It's not like I wanted big boobs or anything. I wanted to be a little bit fuller. I just did it for myself," she told People. In 2020, she confirmed on Instagram that she'd had a second breast surgery. "I encourage anyone who doesn't feel their best self to work on feeling better," she wrote, alongside a photo of herself with her plastic surgeon.

But that's not all Guidice has had done. Speaking at the New Jersey Ultimate Women's Expo in 2019, she told attendees she had a nose procedure. "I did get the tip of my nose done ... So I guess I got a nose job — the tip of it — and I used Dr. Tobias ... He was amazing," she said (via The U.S. Sun).

Clearly, the nose job made quite the difference, because Giudice's physical appearance is strikingly different when comparing a snap of her taken in 2009 and one from 2025. In addition to the narrowing of her nose, Giudice's lips are clearly plumper too, suggesting she may have had filler. However, the star shared in a 2024 TikTok video that she'd dissolved her lip filler, but suggested she wasn't done injecting her pout.

Kim Zolciak's plastic surgery transformation during her time on "The Real Housewives of Atlanta" and beyond has been truly striking. A photo of the reality star taken in 2008 shows just how much she's changed physically, and, compared to a pic from 2025, she looks almost unrecognizable. Zolciak hasn't been coy about her procedures. "I've done my boobs a few times and then I had a tummy tuck. I did have filler in my lips. I do Botox all the time," she revealed on "Botched Presents: Plastic Surgery Rewind." Zolciak also confirmed she had surgery on her neck due to a herniated disc, as well as the skin-tightening treatment Ultherapy.

But despite admitting to plastic surgery on her body, Zolciak (who's been known to wear some cheap looking wigs) has denied getting rhinoplasty. "So you don't think that nose is a little different Kim? It doesn't hang down differently?" "Watch What Happens Live with Andy Cohen" host Andy Cohen asked her in 2016 after showing her a throwback snap of herself. But Zolciak hit back, "No!" before noting, "If I'm going to have a nose job, it's going to be a hell of a lot shorter. It's not going to go down in my mouth."
