Though Kimberly Guilfoyle hasn't ever publicly admitted to getting plastic surgery, anyone who remembers what the Fox News presenter looked like in the early aughts would find it hard not to speculate she's had some work done. Just take a look at photos. Back in 2004, Guilfoyle was a fresh-faced, 35-year-old beauty attending the Princess Grace Awards Gala in a stunning, sheer lilac number. Fast-forward 19 years to a snap she posted to Facebook of herself in a black dress while cuddling her dog, and the staunch Republican looks like a different person.
One of the biggest differences we noticed when comparing the red carpet photo to the social media snap? The size of her lips. The Fox News presenter's top lip looked much larger in 2023. That may be down to different makeup techniques (she's clearly wearing more lip liner in the Facebook upload), but it could also be the result of cosmetic injections. Board certified cosmetic dermatologist Dr. Mariano Busso suggested while speaking to The List that Guilfoyle may have tried lip fillers. Fans have speculated the same, with one X user writing, "Far too much plastic surgery. She looks like a frog or a duck with those lips."
There's also been chatter that suggests Guilfoyle may have received Botox, which can have health benefits, for smoother-looking skin. Board-certified plastic surgeon Dr. Dennis Schimpf gave his two cents, telling Glam, "I think initially she started out with Botox to her forehead, crow's feet around her eyes, and glabella area, as well good skin care." Throw in her much more dramatic makeup and the XXL hair extensions that she seemingly can't live without, and it's no wonder Guilfoyle is almost unrecognizable.

Kimberly Guilfoyle's changing face is just as obvious when comparing a photo taken at a 2004 benefit against a snap she shared via her Facebook two decades later. In 2004, her cheeks didn't appear as full and pronounced, and her makeup wasn't nearly as dramatic. In the aughts photo, she wore shiny, coral lipstick and gloss on the red carpet, subtle faux eyelashes, and barely any mascara on her bottom lashes, which is a far more natural style than we've seen from her in the 2020s (we're begging Guilfoyle to let go of the super dramatic eyeliner and eyeshadow trend). The controversial figure was also sporting a less obvious tan on the red carpet than in her social media snap as she cozied up with her dogs, suggesting the former Fox News personality may not have had such an affinity for faux tan back then.
Of course, people's faces change naturally as they age, and, in an Instagram video, board-certified plastic surgeon Dr. Gary Motykie suggested Donald Trump Jr.'s former fiancé may look different due to intentional weight loss, in addition to the natural fat loss that can happen in our faces as we age. However, Dr. Motykie still suspects Guilfoyle may have had a little help plumping up her face via fillers. Not only that, but the medical professional suggested the political figure may have gone under the knife for something more invasive, stating, "There's a possibility at some point of doing a neck lift or a face lift." Though there's no public confirmation from Guilfoyle that she's had any plastic surgery or fillers, we still can't quite believe she was born in 1969, because Guilfoyle's transformation over the years is truly a sight to see.
